Overview
Women may experience changes in their menstrual cycle at some point in their lives, and while some variation is normal, bleeding that is unusually heavy, prolonged, or irregular deserves attention. In some cases, the problem is related to hormonal imbalances that affect ovulation. In others, structural changes in the uterus, such as fibroids, polyps, or thickening of the uterine lining may exist. In rare cases, abnormal bleeding may be an early sign of a precancerous condition.
Pelvic pain is also common, but persistent or severe discomfort lasting more than a few cycles should not be ignored. Endometriosis, adenomyosis, fibroids, and ovarian cysts are frequent culprits. A careful combination of history, examination, and imaging are often required to identify the source of pain.
Both abnormal bleeding and pelvic pain can affect daily life, work, and relationships, and they may signal an underlying condition that requires treatment. The good news is that in most cases, the cause can be identified and managed effectively.
